Checking In Early: Supporting Patients Over 65 Through Health Checks

Looking after your health doesn’t have to wait until you feel unwell. Sometimes a simple check-in can make a real difference, and that’s what we aimed to do with our recent health checks for patients over 65, as part of our Cohort 5 Project.

We invited 22 patients who are more likely to need extra support from health services. Seven of them took part, with most choosing a home visit, which many found more comfortable and convenient. Some people were already receiving support or felt well, and a few we weren’t able to reach, but each contact helped us understand how to provide care in a way that works best for patients.

During the checks, our team, which included GP trainees, a pharmacist, a nurse matron, and our care coordinator, looked at overall health, reviewed medications, and helped connect patients with any additional support they might need. Our Health & Wellbeing Coach and Social Prescribers were also available to follow up, so no one was left to manage things alone.

The outcomes were encouraging. Of the seven patients who completed a check:

  • Four were advised to follow up with their GP
  • One had a prescription adjusted
  • One booked a blood pressure review
  • Two received referrals to Social Prescribing for extra support

Even these small steps can make everyday life a little easier and help prevent health issues from becoming more serious.

We also learned some important lessons. Most patients preferred home visits over clinic appointments, and reaching people by phone was sometimes difficult. In the future, we may try letters or text messages to make sure more patients can be reached. Flexibility is key to providing care that meets people’s needs.

The project reinforces the value of checking in early and offering support before health problems escalate.
