As part of our ongoing commitment to improving population health management, we developed a Health & Wellbeing Coaching initiative. This project aimed to provide personalised, holistic support to individuals facing complex health and social challenges, empowering them to take control of their wellbeing.
Through this initiative, we supported 64 patients, offering tailored interventions to improve their overall health, motivation, and quality of life.

One impactful case involved a 61-year-old male patient who was struggling with:

✔ Connected him with Gamblers Anonymous for addiction support

💡 Health & Wellbeing Interventions:
✔ Conducted a diabetes review and developed a management plan
✔ Arranged physiotherapy for both him and his partner
✔ Organised an occupational therapy assessment for mobility support
✔ Encouraged medical consultations, leading to a Parkinson’s diagnosis for early intervention
✔ Requested a learning difficulty assessment to ensure long-term tailored support
