Integrated Neighbourhood Team (INT)

One of our core principles is that, where possible, care should be delivered as close to your home as possible. Our commissioners want to provide the best care across the Mid and South Essex area, but we know that some localities have different requirements. For example, someone living here in Benfleet, Hadleigh or Thundersley may have very different health and care needs to those in, for example, the centre of Southend City or out in the Dengie area of mid Essex.

How does it work?

The integrated neighbourhood teams (INT) work with wider partners in their local area, making sure that wider issues of health and wellbeing can be addressed. These partners include local councils, doctors’ surgeries, hospitals and local voluntary and community groups all working together. Each INT serves a population of between 30,000-50,000 people.

These INT build on Primary Care Networks (PCNs) that have existed in our area for some time.

Programme and project managers work with the local partners to develop these teams, looking for better ways to work together and improve people’s experiences of care.

What does it mean for me?

We want our people to live healthy, resilient and independent lives, with simple access to support and care when needed, as close to home as possible. We will organise care with, and for, the people living in our communities so that they experience the health and wellbeing outcomes that matter to them.​

This means that you will be able to talk to people about your health and care in a place that works for you, in the way that works for you. We will also work with you to ensure that you are involved in and can influence how and where you receive care. Our Partners

District Nursing
The district nursing service offers nursing support for housebound patients and their families to aid and support clinical needs in the patient’s own home.

People can be referred to the service by their GP or other multidisciplinary teams.

Dementia Intensive Support Team (DIST)
DIST provides intensive support to patients with dementia of any age and will attempt to prevent hospital admissions to both acute and psychiatric hospitals.

Primary Care Mental Health Team: South East Essex
Mental Health Professionals embedded into your GP surgeries and allocated via the Primary Care Networks.

Palliative Care Nursing Team
Palliative care is for people with advanced and progressive illness that cannot be cured. The Palliative Care Nursing Team consists of nurses who have undertaken specialist training and have experience and skills in palliative care.

Service provided in patient’s homes including residential care settings. Castle Point, Rayleigh and Rochford Team is based in clinics in Canvey, Thundersley and Hockley.

Care Coordination Team
The Care Coordination team provides a thorough review of the health and social needs of patients with long-term conditions or frailty. Helping them in accessing services available in South East Essex.

Anyone can refer to them, including the patient themselves.
